Infection hygiene audit

Helping hygiene infection nurses conduct audits and report their findings from them digitally and easily improves the quality of the audits, their usefulness and helps save substantial amounts of time. During audits but in particular in writing reports, which means report recipients receive reports faster – often immediately after the audit is completed – leading to improved hygiene in hospitals and nursing homes. And thus, in turn, likely to less sickness and deaths.

Hand hygiene audit

Whether interview-based or observation-based (5 Moments), QReport can be set up to support data collection for auditing for both approaches. In the interview-based model, key department hygiene representatives perform the audit by simply asking department staff question regarding hygiene compliance and habits. In the observation-based model, the auditor will observe and monitor how staff act and record non-conformity / conformity in the app along with the type of task/process being done. For both approaches, data can be visualized in real-time in dashboards, available online and directly in the QReport mobile app.

Environment and hygiene inspection in day care

As a major potential source of infections and spreading of infectious disease, day care centers have a lot to benefit from addressing hygiene and environmental issues, whether for staff, children, or parents. Regular reviews that facilitate dialogue, learning and prioritization are an excellent approach to managing the fairly complex challenges faced in day care centers. QReport provides health nurses with a mobile solution that both offers a structured, quality-improving overview that enables easy recording of relevant issues, and a comprehensive guide to the rules, general and local guidelines and best practices that should form the basis of the review and subsequent activities, and dialogue between nurse and day care center.

INSTA 800 / NIR - Audits for cleaning quality and hygiene

Applied in several countries, INSTA 800 is a quality standard for cleaning in hospitals and across the primary care sectors, where focus is on collecting data on a mix of quantifiable impurities and qualitative measures that need to be managed extra carefully in the health care sector. NIR is a National Danish infection Hygiene guideline for hospitals and the primary care sector, focused on minimizing potential infection risk, and thus disease, sickness, and avoidable deaths. INSTA 800/NIR audits in QReport work with stratified sampling across classified premises and types of rooms found in hospitals, nursing homes etc. and the auditor is assisted with automated responses for both deeper investigation and audit scores. In addition to improving audit quality and efficiency for auditors and relevant staff, QReport helps provide actionable data and a valuable overview to guide efforts to improve hygiene and cleaning in health care sector. Optionally, nonconformities discovered can trigger actions from responsible cleaning staff to ensure hygiene or quality issues are managed and addressed in a structured way.

Pressure sore prevalence audits

Besides being painful and potentially deadly, pressure sores can pose a tremendous burden on hospitals, justifying serious attention to mitigation and avoidance for less-mobile patients. With QReport as the mobile audit data collection solution for pressure sore audits, auditors are able to simultaneously work qualitatively and quantitatively when auditing departments and examining patients during the auditing process. Subsequent reports and data are quickly and efficiently generated and distributed for rapid action plan execution and optimal knowledge sharing between hospital staff.

Audit på arbejdsmiljø risikofaktorer og ISO45001

Hospitals are large organizations with inherent exposure to many risk that are not found in other sectors; Audits are conducted on based on defined occupational risk factors (E.g. Physical, Ergonomical, Biological, Chemical, psychological, organizational, and accidental) for types of premises/rooms, activities where they can appear, wherein the risk lies, and how risks are prevented. In QReport, these factors – defined in a database called Risava – are combined with ISO 45001 (OH&S) and built on situational logic to constitute an exceptionally comprehensive audit software tool that is simultaneously easy to use. All data is collected in a ‘ready-to-report’ structure, leaving only minor quality assurance adjustments to be required before reports are distributed to relevant parties.

IT Information security audits

QReport will be the data collection tool when the IT-departments conduct Information security audits to evaluate cmpliance with information security policies and rules. Reporting in QReport to happen immediately after the audit is complered and reports are to be immediately sent to responsible department leaderhips automatically

Radiation Protection audit

As part of an EU initiative, hospital radiation (Radiology, X-Ray, nuclear medicine, oncology, etc.) protection audits have been deemed mandatory with the aim to structure the efforts towards patient and health care personnel safety in radiation-prone hospital environments. Data collection via QReport is to ensure data is captured easily and in a high-quality-focused, secure, structured, and standardized manner, opening the floor to data and knowledge sharing across hospitals. Reporting via automatically generated outputs distributed directly to relevant recipients. Collected audit information data can be visualized in real-time in dashboards, available online and directly in the QReport mobile app.

Audit for Blood sampling, EKG, Biochemistry, and immunology

Observation-based audits for EKG and blood sampling to monitor adherence to defined processes and enforce the principles that need to be followed by hospital staff tasked with these activities. In QReport, all observations and subjects are recorded, and both progress and results are visible real-time, in both QReport web and mobile applications, providing observers/auditors possibility to react instantly to observed behaviors. Periodic and follow-up reports can be compiled and generated by users immediately by request.

Waste / Hazardous waste audits

A significant share of hospital waste needs to be meticulously sorted into fractions before being discarded or recycled to ensure E.g. dangerous waste is handled properly. Supporting an observation-based approach, QReport provides the waste inspector with a tool for easily registering observations and summarizing findings into conclusions that can be reported instantly with minimal administrative effort when the audit is completed. Additionally, observation data collected can aid analysis of waste composition etc.

Internal audits for Cleaning and Hospital Services

For the miscellaneous types of areas to audit within the overall cleaning and Hospital Service frame, the flexibility built into QReport vis-avis the app template designer staff with functional expertise are enabled to set up their own audit and reporting mobile applications. This improves the quality and specificity of audits whilst saving time and effort that can be directed towards other purposes.
